This 2-1-1 Call Center is a 24-hour hotline call center in San Joaquin County that offers comprehensive referrals to community services and resources. This program supports families with children ages 0 to 5 by increasing the number of families that are screened and connected to services and resources. Additionally, 2-1-1 Care Coordinators also provide families with children ages 0 to 5 with family needs assessments, individualized case support, and facilitate connections to resources and services for up to 30 days.
